Saint Joseph’s Patient Exchange is a free, secure way to see some records of your hospital stays at Saint Joseph’s Medical Center since July 1, 2014. It is a convenient way to keep track of your medical history and share it with others, if you wish. Only patients age 18 and older may sign up for Saint Joseph’s Patient Exchange and give access to others, including their medical providers.

To sign up for the Saint Joseph’s Patient Exchange, you will need a validation code that was given to you at your admission. For security reasons, the code expires after seven days. If you do not have your code, or if it has expired, please email patientexchange@saintjosephs.org for assistance.
Saint Joseph’s Medical Record Access
The Health Information Management Department at Saint Joseph’s keeps documentation of each patient’s medical care during hospitalization. The medical record includes test results, medications, treatments or therapies, x-rays, and reports.
You may obtain a copy of your medical record in the following ways:
- Log into the Saint Joseph’s Patient Exchange online portal.
- Contact the Health Information Management Department at the hospital with your request. You will be asked to complete a release of information authorization.
