Health Home Care Coordination

Health Home Care Coordination offers eligible clients with complex behavioral and medical conditions who are insured by Medicaid a variety of services, including:

  • Assistance and coordination with medical and psychiatric appointments and providers
  • Help with financial entitlements
  • Support for vocational, educational and socialization goals
  • Assistance with housing

Care coordination staff can provide linkages to services and agencies throughout New York City and Westchester County to assist clients in avoiding re-hospitalizations and unnecessary use of emergency services.

Clients who do not have a behavioral health diagnosis, but who have certain medical conditions may be eligible for health home services. Those who do not have Medicaid are eligible for care coordination services if they have a diagnosis of a serious mental illness.
