Access to Medical Records
To request that a copy of your medical records be sent to another provider, please call the St. Vincent's Medical Records Department at (914) 925-5251 to obtain a release and consent form. Once a completed form is received, the request is usually processed within 48 hours. Requests concerning minor patients must be filled out by a parent or guardian.
If a patient requests records for him/herself, a patient access form must be filled out and approved by the treating physician before the records can be released. Please note that there is a photocopy fee associated with these requests. It can take up to two weeks to process these requests.
