Saint Joseph's Medical Record Access

The Health Information Management Department at Saint Joseph's keeps documentation of each patient's medical care during hospitalization. The medical record includes test results, medications, treatments or therapies, x-rays, and reports.

You may obtain a copy of your medical record in the following ways:

  • Log into the Saint Joseph's Patient Exchange online portal at  https://portal.saintjosephs.org.   Click here for more information.
  • Contact the Health Information Management Department at the hospital with your request.  You will be asked to complete a release of information authorization.
